Students are supposed to download the official application form for the scholarship and fill it with relevant details. The application form is to be submitted before the last date.
The students along with the form also have to submit an affidavit on a 50Rs. stamp paper of the student or the parent which should be signed by an SDM, a gazetted officer or any First Class Magistrate. Also the required documents are: 

One copy of passport size photograph
Attested certificates of educational qualification
Income declaration – affidavit
Address Proof
